This form must be completed and submitted to Carolina Imaging Products before an RMA number can be issued. Your RMA number will be emailed to you once the return is approved. It will include instructions for returning the merchandise. No credit will be issued if product is returned without an RMA number. Please see terms & conditions for return policy details.
